PATIENT RIGHTS

 

This page outlines specific rights and standards of staff behavior afforded to each CCC patient. The policies below are listed because the federal government has specifically mandated that we must "post this [information] prominently in waiting areas, as well as any other locations accessible to clients or potential clients." By posting these policies, we do not imply that these are any patient's only rights in regard to interactions with the CCC.

 

 

 

PATIENT PRIVACY POLICY

 

Effective April 14, 2003

 

The entire staff at the Comprehensive Care Center is very concerned about the privacy of your personal health information. We are fully dedicated to protecting information about you. We create records of the healthcare and services you receive at the Comprehensive Care Center in order to provide you with the highest quality of care and to be sure that we meet certain legal requirements. Our Notice of Privacy Practices that follows here will tell you about the ways we are allowed to use and disclose information about you and to make sure that you are informed about your rights and our obligations regarding the use and sharing of your personal health information with others. The law requires us to keep private any information that identifies you. We are also required to give you this notice concerning our legal duties and privacy practices as they apply to your protected health information. We must also be sure to follow the terms of the privacy notice that is currently in effect.

NON-DISCRIMINATION POLICY

 

The Comprehensive Care Center provides considerate, respectful, and dignified care to all clients without regard to age, race, sex, handicap, religious beliefs, national origin, sexual orientation, or source of payment for treatment.

 

In addition, the Comprehensive Care Center affirms that all civil rights, including those regarding freedom from discrimination based on age, HIV infection or AIDS, and handicaps are protected under Title VI of the Civil Rights Act of 1964, the Age Discrimination Act (ADA) of 1975, Section 504 of the Rehabilitation Act of 1973, and the Americans with Disabilities Act.
